Registered Nurse Evaluation

An Alternative Home Care Registered Nurse (RN) provides a comprehensive health and medication evaluation addressing the patient's physical, emotional and social needs, as well as identifying the personal and homemaker support and services needed. The assessment is then used to develop an individualized service plan and provides a guideline for further assessments. The RN Case Manager schedules the assessment visit with both the buying and receiving customer. NOTE: Licensed services including medication set-up and management personal care visits and nursing visits must be preceded by a health and medication evaluation.

The Evaluation Includes: A safety review of the home for any visible hazards or other home maintenance issues. A complete physical assessment including current medications. A Registered Nurse (RN) Case Manager will contact the senior's physician to verify medications, treatments and physician orders. The Registered Nurse (RN) Case Manager will contact the customer to review these findings, discuss the individualized service plan and schedule services.
